In the spirit of transparency, The Empowered Patient Coalition is releasing a preliminary summary of aggregated data from our medical harm reporting survey. event so the number of incidents reported is greater then the number of individual responders. more data is collected it will include more specific details, including the factors that patients have identified as contributing to their incidents.
