Personalize your own medical experience or create a patient safety video in honor of a loved one. Sharing your stories will help others find the resources they need to stay safe in the health care system.

A video can be customized in a few easy steps and EPC volunteers will assist you if you have any questions.

Please provide a photo(s) and a brief dedication and then choose a patient safety theme (Being an Empowered Patient, Safe Surgery or Preventing Medication Errors) and music. Our volunteers will create your video, put a link to the video on our You Tube channel and then email you the direct link. You can honor your loved one, pass along important patient safety information and help us further our mission by sharing the video with your friends and family and posting to social media websites such as Facebook. Please ask others to forward your message so we will reach as many people as possible.

About This Project

The idea for this project came from advocate Becky Martins who has provided peer support to patients and their families after they have experienced medical harm. People are often searching for a way to make their own contribution to the patient safety movement and are willing to share their experiences to help others. A video was conceptualized that would honor an individual, provide patient safety tips and then direct viewers to the many resources available on the Empowered Patient Coalition website. We have worked diligently to ensure that Becky’s innovative vision becomes a reality.
