Hi, my name is Giovanna, I live in Vancouver B.C. and want to make a documentary on "Preventable Medical Errors in Hospitals." If there are any of you out there who have suffered an adverse event in a Canadian or American hospital, a Care Facility, or someone you know died as a result of an adverse event or medical error, I want to hear from you.
